Braylynn S. Fahy, a 30-year-old woman from Fort Plain, has been identified as the driver killed in a serious collision Monday evening on the New York State Thruway near Exit 24 in Albany, according to New York State Police.

The crash occurred around 6:45 p.m., when Fahy’s 2018 Chevrolet Traverse merged onto Interstate 90 westbound from the Exit 24 acceleration ramp off I-87. Investigators say Fahy’s SUV crossed all four lanes of travel moments after entering the Thruway and was struck from behind by a 2025 Cadillac Escalade traveling in the left lane.

Fahy suffered critical injuries and was rushed to Albany Medical Center, where she was later pronounced dead.

Multiple Passengers Injured

State Police said two adults and three minors were inside one of the vehicles involved, while four adults were in the other. All passengers were treated for non-life-threatening injuries at local hospitals. No additional fatalities have been reported.

Investigation Ongoing

Troopers are continuing to review what caused Fahy to cross multiple lanes of traffic immediately after merging. Authorities have not released details regarding speed, impairment, or other contributing factors.

The section of the Thruway near Exit 24 was temporarily shut down as first responders, crash reconstruction teams, and emergency medical crews worked at the scene.

State Police say the investigation is active and that updates will be released as more information becomes available.
